____ $5 weekly tape/CD, mailed weekly

____ $15/month, 4 tapes/CD's mailed monthly

____ $40/3 months, 4 tapes/CD's mailed monthly

You may also specify certain tapes/CD's from specific occasions or dates if you wish for $5/tape/CD.
